Your BER assessor will have emailed it to you. It's also available at ber.seai.ie — search by address or BER number. The file is titled "Dwelling Details Report".

How much of the external walls are windows?
Most Irish homes are Low or Typical. A house with a large rear extension or lots of glass would be High. Not sure? We'll assume Low (15%) unless you pick a different option.

Wall insulation options explained
CWI means cavity wall insulation, injected into the gap between the walls. IWI means internal wall insulation, fitted to the inside face of the wall. EWI means external wall insulation, fitted to the outside of the wall.
